AP Exam ID Form
All AP Exams Require a Valid Photo ID to Test.
For every AP Exam, all students need to present a recent, valid photo ID or the AP Student ID Form.
Acceptable ID Examples:
- Valid Government-Issued Driver's License
- Valid Learners Permit
- Government ID
- Passport
- Completed and notarized AP Student ID Form
- Use the link to the AP Student ID form to download the document.
- Follow the instructions on the form (except for the school's official information) and attach the appropriate photo type according to the guidelines on page 2
If a student DOES NOT have one of the above-listed photo IDs 1-4, then they must complete an AP Student ID Form and have it notarized in the main office by Mrs. Baker.
Well before your first exam, you should bring the completed AP Student ID Form, which must be verified and notarized by Mrs. Baker in the front office.
You must bring the notarized and completed form to each exam you take at AAI in place of a driver's License, school ID, passport, or Learner's permit..
